Authorization, deductibles, and copays

Pre-authorization. Most Cigna plans require authorization for detox, inpatient, and PHP. Our utilization review team submits the clinical justification and typically has approval the same day. Continued-stay reviews happen every 5 to 7 days for inpatient care; we handle those too.

Deductible. If you have not met your annual deductible, treatment costs count toward it. Many clients who enter treatment late in the year have already met it.

Coinsurance and copays. After the deductible, you pay a percentage (commonly 10 to 30 percent for in-network) until you reach your out-of-pocket maximum. After that, the plan pays 100 percent for the rest of the year.

Out-of-network. Cigna PPO plans usually include out-of-network benefits at a higher coinsurance. Where a plan has no in-network option that fits, we pursue single-case agreements.
